First you'll need to download the Quick Map Wizzard application, you can download it right here:Quick Map Wizzard V1.02.
Quick Map Wizzard requires the .NET 2 framework to run
1) Extract the zip to a folder.
2) Run MapWizard.exe -

3) In 'game path' type in the path to a folder to save the files in (e.g. F:\QW\base). To be safe, don't use your actual retail game folder, but make a copy or use the SDK base path. For testing, the files should be zipped up into a pk4 and placed in the game 'base' folder.
4) In 'Map filename' type in the short name of the map. This is used to generate the various files (in Mordenkainen's tutorial it was 'tut1')
5) In 'Map name' type the name of the map as it should appear in game ('Tutorial 1')
6) In thumbnail levelshot, type in the thumbnail image to use for the map ('area22') - this gets put into pakmeta.conf as the tga file to use for 'server_shot_thumb')
7) Click 'OK' and it should make the skeleton map files and folders.
The 4 settings are saved in the 'MapWizard.exe.config' file.
It creates the following files:
pakmeta.conf
script\main.script
script\maps\<mapname>.script (blank)
maps\<mapname>\<mapname>.world (empty)
mapinfo\<mapname>.md
